By registering to take part in Heart Week activities, I agree to properly wear a mask at all times, maintain social distancing and follow other safety protocols set forth by MHS administration and staff. Hand sanitizing will be required upon entry and exit. Temperatures will also be taken at the entrance.

If I do not follow safety protocols, I may be asked to leave and will not be given a refund of my ticket purchase.
