Waiver of Liability

This agreement releases Bangor Township Schools and John Glenn High School from all liability relating to injuries that may occur during the John Glenn High School Youth Basketball Camp, located at John Glenn High School 3201 Kiesel Rd. Bay City, MI 48706. By signing this agreement, I agree to hold Bangor John Glenn High School and its staff/employees entirely free from liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.

I also acknowledge the risks involved in playing basketball or completing basketball drills and my child is physically fit to participate in this camp. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, my son does not have any conditions that will increase his likelihood of experiencing injuries while engaging in this camp.

By registering my child for camp, I forfeit all right to bring a suit against Bangor Township Schools and/or John Glenn High School for any reason. I grant permission for photos and other records of this event to be used for promotional purposes. If my child requires medical attention, I hereby give consent to authorize emergency medical personnel to provide such medical care as deemed necessary. I also understand that as a participant in this camp, I agree to ensure my child conforms and complies with all laws of the United States, the State of Michigan and all ordinances of the city of Bay City, MI and all the regulations of John Glenn High School.

All issues relating to the sale of tickets and the hosting of their associated event(s) are the responsibility of the “Event Organizer” and not that of Revtrak inc d/b/a Vanco Events.
