By submitting this online registration form, I, as parent/legal guardian, hereby grant permission for my teen to participate in this event that requires transportation to a location away from the parish site. I understand that this activity will take place under the guidance/direction of parish employees and/or volunteers from the parish. I remain legally responsible for any personal actions taken by my teen & agree, on behalf of myself, my teen, or our heirs/successors/assigns, to hold harmless and defend Saint John Neumann Catholic Church, its officers/directors/agents & the Archdiocese of Atlanta, GA, chaperones/representatives associated with the event, arising from or in connection with my teen attending the event or in connection with any illness/injury/cost of medical treatment in connection therewith, & I agree to compensate the parish, its officers/directors/agents, and the Archdiocese of Atlanta, chaperones/representatives associated with the event for reasonable attorney's fees & expenses arising in connection therewith.
