Parent/Chaperone acknowledgement:

By purchasing this parent chaperone ticket, I am agreeing to attend this event as a chaperone that will be responsible for driving at least 4 assigned students in my car, monitoring student behavior before, during and after the event and returning those same 4 students in my car at the conclusion of the event. I acknowledge that there will be paperwork to fill out providing proof of insurance and driver's licenses as well as student and chaperone expectations to be held accountable to. If I am unable to provide these accommodations or complete this paperwork in a timely manner, I am forfeiting my ticket to be given to someone else.

Student ticket acknowledgement:

By purchasing this student ticket, I acknowledge that I will be driven in a parent chaperone car that will be assigned to me by Ms. Armstrong. I am also acknowledging that I must abide by the Laredo Middle School code of conduct and expectations assigned by Ms. Armstrong. I am a representative of Laredo Middle School when attending school organized activities and must behave in the most respectful manner toward my peers, adult chaperones and other patrons of the DCPA, otherwise I will not be invited to attend future Drama Club events.
